Tidied up the userland/kernel interface so that all the data transfer happens
in the read/write path rather having some copy_*_user side-effects.
Also tidy the dlm_*_wait[] calls so they share code with the normal calls.

This breaks kernel/userland compatibilty. You must upgrade both kernel and
userland [libdlm] together!
